Birth Registration & Certification of Birth

Important Notice:

Please note that Certification of Birth is not Birth Certificate. For Birth Certificate, please reach out to the place of  birth and in case of birth in Pakistan, the respective Union Council or City Office can issue new or duplicate Birth Certificate.

Requirements for Birth Registration (S-1 registration could only be processed for children under two years of age):
  • Complete Form S-1 (click here to download) 
  • 2 current passport size photographs of the child from front view with white background.
  • Notarized Birth Certificate.
  • Proof of father’s or mother’s Pakistani nationality (Notarized copy of CNIC/NICOP or Passport). In case of different nationality of any parent, please provide the concerned parent’s copy of passport.
  • State ID/Driving License of both parents.
  • Fee ($20) payable by debit/credit card if appearing in person or money order if being sent my mail.
  • Self-addressed prepaid return envelope of your preferred courier service with a tracking number.
Requirements for Certification of Birth
  • Original Pakistani Passport and Pakistani ID Card (CNIC/NICOP) along with one photocopy of each document.
  • To add the mother’s name on the Certificate to be issued, submit applicant’s Birth Certificate or mother’s Pakistani passport/CNIC/NICOP containing husband’s name along with one photocopy.
  • Copy of US State ID or Driver’s License.
  • Fee ($20) payable by debit/credit card if appearing in person or money order if being sent my mail.
  • Self-addressed prepaid return envelope if the application is being sent by mail.
