Applications from Indian Nationals residing on permanent residential status in the following states are eligible to apply at the Consulate General of Pakistan in Chicago: Illinois, Wisconsin, Minnesota, Iowa, Missouri, Indiana, Kansas, Michigan, Nebraska, Ohio, North and South Dakota
